Just leave your phone turned off on vacation. It's vacation, they can't ask you to work.

Non of this matters. You are on vacation, you are not on call. There are labor laws on this.

Turn off the phone. Or ignore the calls. If they turn you down for coverage that's on them not on you.

Yeah PTO is not just "time not in the office" its time that you are working. Its a part of your pay, and there are very strict laws about being able to use it, along with conditions of what pay you should be receiving if you're forced to work on your vacation.
It's on the business to get the issue(s) resolved while you're away.

But they are not doing their jobs properly because you are willing to give up your vacation to protect them from their mistakes. That's not good for you, not healthy for the profession. Yup, which is why based on @stess current assessment, we probably know that this wasn't a legit decision before, or there would be no issue implementing it now.
