VM from ESXi to Xenserver

Ignore me. Closing XenCentre and reopening seemed to work

Ya that issue has been around for a while. Not sure what the problem is. Sometimes if you just switch tabs or to a different VM and back it will show up.

Did you remove esxi tools from guest before export? This is important.

Migrating from VMWare to Hyper-V, I have never found this to be required. In fact, it causes more issues many times.
I typically remove the VMWare tools afterward.

Did the same (vSphere 5.5 -> Hyper-V 2012 R2), just remove them past-import.

How to from Citrix on exporting/importing from esxi to xenserver.Of course it is not. But honestly, that part is not relevant. He is migrating V2V. The assumption that the VMWare tools have to be removed prior to a V2V is generally false. Depending on how your VMs are setup and what OS they are running you can very easily break them by removing the VMWare tools. This would then prevent the V2V.
I do not have a specific answer for the OP, but I provided as much information as I could always stating that the issues I dealt with were from a VMWare to Hyper-V V2V process.
Specifically to the CentOS test, the boot image looks like it needs rebuilt prior to the V2V or from a boot media after the V2V.

This is what i'm thinking at the moment but unsure how to do this with my level of Linux skills.
