mps0: Warning: io_cmds_active is out of sync - resynching to 0

Getting this on a SuperMicro chassis that we are trying to do an install on.
Anyone know what is giving this error? I've not started researching yet as I was just told about it and wanted to get it posted so that we had a place to work.

My guess is hardware RAID failure, but just a hunch so far.

It's a pfSense install, so FreeBSD 10.1
On old hardware?
Yes. Not real old, but oldish. It's a quad core Xeon, 12GB RAM. LSI Hardware RAID.

Yeah my $10 goes first to failed LSI card, second to loose cables or several toasty drives. Anyone have eyeballs on the drives in the last week or two? Could have had one dead for a while and another just packed it in.

I had a suspicion that the firmware might have been an issue from another thread that I found. That seems likely.

This is currently postponed. A lot of systems being built tonight. Don't know if we will get back to this one right away of not.
